Physician Retirement Letter

A physician retirement letter is an opportunity to notify patients of your retirement and to thank them for allowing you to tend to their medical needs. A physician holds a position of trust. Honor that trust by sending the letter in a timely fashion.

Format and Content:

The content of the letter should include information that patients will need both before and after your retirement. The first paragraph of the physician retirement letter format provides clients with the date of your retirement and makes a suggestion regarding future medical care. The second paragraph provides additional helpful information. The third paragraph contains information about your patients’ confidential medical records. Close the letter with a genuine and thoughtful thank you.

This sample physician retirement letter is written by a doctor who is leaving a group practice. The tone is professional and friendly. The physician recommends his patients choose another physician in the practice. He takes the time to direct them to helpful resources and provides them with contact information if they have questions.

Dear Patients,

I have enjoyed caring for you and your families over the years. With a lot of joy, and some sadness, I am writing to let you know of my intention to retire. My last day with Columbus Family Practice will be on August 31, 2013. As you all know, this practice is staffed with some of the most knowledgeable and caring physicians in the country. I highly recommend that you select one of these physicians for your future medical needs. You can review the bios of all of our staff by visiting our website at www.Columbusfamilypractice.com/physicianbio.

We understand that some of you may want to select a new physician outside of our practice. Please be assured that my staff and I will do everything we can to make the transition smooth and stress free. I recommend that you begin looking for a new physician as soon as possible. You may contact the physician referral line at local hospitals or you may use one of the many online physician referral systems.

Your medical records are confidential and will remain on file with Columbus Family Practice. If you choose another physician within our organization, you do not need to take any action concerning your records. If you choose a physician outside of our clinic, you will need to sign an authorization form before we can release your medical records. Please note, by law we cannot share your medical information without your written consent. You may sign this form at our office or you may complete the necessary paperwork at your first visit with your new doctor.
