Medical Leave Letter

When employees need to take time off from work in order to care for their medical needs or those of their family members, it is professional to send a medical leave letter to employers. The simplest medical leave letter format should include a brief summary of your upcoming medical procedure and the dates you will need to take your leave, clearly stated in the style of a typical business letter. I appreciate your assistance in this matter as I will need the two weeks off work in order to be fully recovered so that I may be more focused on my work when I come back. Enclosed with this letter, you will find a note from my doctor confirming the date of the procedure.

I would like to extend my assurances that I will strive to complete all projects I am currently working on before my surgery, and put forth as much effort as possible to make up for the lost time once I return to work.
